The contractor operating the excavation equipment should complete the <br /> enclosed "Contractors Questionnaire" and provide proof of workers <br /> compensation, if required. <br /> 8. The "Chain of Custody" and 3/9/89 w ter sample results are missing from <br /> report. <br /> These concerns shall be addressed in a re iced work plan prior to continued <br /> field investigative work.
