i _ _ Fs,. TERMS AND CONDITIONS 1991 RATE SCHEDULE--GROUNDWATER TECHNOLOGY,INC. Terms: Paymen'is due within 30 days upon receipt of invoice. A finance charge of 1.5% per month will be assessed for amounts past due. Rates: Rates include normal accounting time,telephone charges,computer usage,postage,copies of reports,bindings,facsimile transmission costs, and routine copying of documents.Special project specific accounting time will be billed at the appropriate hourly rate. Other unusual expenses will be billed at cost plus 20%unless otherwise specified. Special request services will be charged per individual contract. Subcontractors'services and expenses will be billed at cost plus 20%. Supplies: Hardware,repair parts and other items will be billed to specific projects according to the local Groundwater Technology, Inc.Standard Material Price List which is available upon request. Items not identified on the materials price list will be billed at cost plus 200 Market availability and supplier price fluctuations may cause prices to change without prior notice. Automobiles: Automobile mileage is charged at the prevailing applicable IRS rates. Trucks: Company pickup truck mileage is charged at S.60 a mile,with an additional S251day charge far trucks used for 0 to 4 hours per day per project. For pickup trucks used over 4 hours per day per project, there is a full day charge of$451day, plus S.601mlle charge. For use of all other vehicles,frr 0 to 4 hours a day, there is a daily charge of$45lday, plus$.60 a mile;for use over 4 hours a day,there will be a charge of$80/day plus$.601 mile per day per project. Vehicle charges are subject to change without notice. Travel: Out-ef-town travel(e.g.,meals, lodging,car rental,airfare)and other direct travel expenses will be billed at cost plus 100/0. Overtime: Delivery of depositions, responses to subpoenas or expert testimony will be billed at 1.5 tures Fee Schedule Rates. All overtime for support personnel(more than eight hoursiday)will be billed at 1.5 times Fee Schedule Rates. All weekend and emergency response for both professional and support personnel will be billed at 2.0 times Fee Schedule Rates. Lab 1 Equipment: Any services of GTEL Environmental Laboratories, Inc.,or equipment manufactured by the ORS Environmental Equipment division,will be billed at current rates and list prices which are available upon Effective 1/28191 request.
