The following is an itemized list of hazardous materials violations that have not been <br /> addressed for ADVANCED TRAILER SALES & SERVICE as of September15, <br /> 2020. <br /> Open violations from August 03, 2020 inspection <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan for 2018, 2019, and 2020 and the last map <br /> submittal was not legible and did not show/address loading areas, internal roads, evacuation staging area, <br /> emergency response equipment(fire extinguishers and spill control), emergency shutoffs, and access/exit points, <br /> and the locations of argon/carbon dioxide mixture, used antifreeze, motor oil, and propane. A site map shall contain <br /> a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br /> "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#7 -Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures have not been submitted for 2018, 2019, and 2020 and the emergency <br /> response procedures were lacking the correct contact information for the Local Unified Program Agency. Section C. <br /> the phone number for the Local Unified Program Agency,209468-3420 has not been identified. A business plan <br /> shall include the following emergency response procedures for a release or threatened release of hazardous <br /> materials, including, but not limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#9 -Failed to annually review and certify that the business plan is complete, accurate,and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually for 2018, 2019, and 2020. The hazardous materials inventory shall be submitted by <br /> January 15 of each calendar year and may be submitted beginning November 1 of the previous year.
