Please complete the following: <br /> _ Results of previous investigations. <br /> The stamp or seal of a registered geologist or certified California geologist, or <br /> registered engineer (State of California registration). <br /> _ Detailed site map showing tank location and type, piping, streets and adjacent properties <br /> (north toward the top of the page). <br /> _ Results of tank tests (including type of test). <br /> _ Estimates of quantity and compositions of the liquid leaked (including basis of estimate). <br /> _ History of site use (including tank use). <br /> _ Local topography, geology, nearby streams, estimated or measured depth to groundwater, <br /> _ Location, depth, and type (municipal, domestic, irrigation) of water wells within 500 feet. <br /> _ Location of nearby septic tanks, leachfields, buildings, and underground public utility <br /> lines (including water, sanitary sewer and storm sewer). <br /> _ Proposed program, including methodology, to define the extent of soil and groundwater <br /> contamination. <br /> Proposal to dispose of soil boring(s) cuttings and/or bailed water volumes from monitoring <br /> well(s). <br /> Lithologic logs have not been signed by the registered geologist or certified engineering <br /> geologist (State of Calif. registration) who is responsible for logs' accuracy. <br /> _ Depths to groundwater; shallow, unconfined and confined zones. <br /> _ Elevations of monitor wells and/or domestic well(s) on property. <br /> _ Groundwater gradients based on periodic water level measurements from on-site monitor wells <br /> and domestic well(s). <br /> _ Lithology and cross sections based on boreholes, trenches, and other supporting <br /> information. <br /> _ Proposal for additional boreholes and/or monitor wells necessary to define the exter.': of <br /> contamination. <br /> _ Soil _ Groundwater <br /> XX_ Other LABORATORY METHODS ARE NOT ACCEPTABLE.
