0 41 <br /> Appendix A <br /> • Sample No: Sequential from 01,for areas that have two or more similar sample types from the same <br /> site <br /> • Sample Depth: h1 Parentheses measured in feet bgs. <br /> Thus, a sample with an ID number ofNBPEXFL2(14)is floor sample No. 2 at 14 feet bgs,of the <br /> excavation at the North Burn Pit. <br /> Sample ID numbers will be used on sample labels,field logbooks,sample tracking forms,COC records <br /> and any other documentation applicable to sampling activities.The sample ID umbers will also be used <br /> by the laboratory for sample data reporting. <br /> All information pertinent to the environmental samples,including specific fiel collection data,name of <br /> the sampler,and observations will be recorded in permanently bound notebooLs. Sample IDs will be <br /> linked to the site where the samples originated. The laboratory will also empl y a laboratory information <br /> management system(LIMS)to assist in tracking the progress of each sample I hrough the analytical <br /> process. <br /> A1.5.2 Sample Packaging and Shipment <br /> Samples will be sealed in ZiplocTm bags and packed in a cooler to keep them eparated from one another <br /> and dry. Ice, sealed in plastic bags,will be added to the cooler in sufficient qi Lantity to keep the samples <br /> at a temperature of 4 t 2°C for the duration of the shipment to the laboratory. <br /> The COC form will be completed by field personnel,and released to the laboi atory courier via signature. <br /> The cooler will then be released to the courier for transportation to the laborai Dry. If a commercial carrier <br /> is used,the COC form will include the air bill number in the"transfers accep d by" column and will be <br /> sealed in a ZiplocTT"bag. The COC form will then be taped to the inside of ff a sample cooler lid. The <br /> cooler will be taped shut with strapping tape. <br /> A1.5.3 Field Documentation <br /> At a minimum, sampling information will be recorded on a COC form and in the field logbook. Both <br /> documents will be completed in the field at the time of sample collection. Al entries will be legible and <br /> will be recorded in indelible black ink. <br /> A1.5.3.1 Chain of Custody (COC) Record <br /> At a minimum the following information will be recorded on the COC form: <br /> • Projectname <br /> • Project location <br /> • Projectnumber <br /> • Client representative <br /> • Project manager <br /> • Sample numbers,including depth <br /> KB61125 APPENDIX A MACTEC Engineering and Consulting, Inc.
