S0 JOA U 1 Environmental Health Department <br /> u ; i y I AMENDED <br /> 6/18/2020 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Walgreens#12549 2040 E Mariposa Rd, Stockton June 17, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> OBSERVATION: At the time of inspection, it could not be demonstrated (not necessarily documented)that <br /> employees who handle hazardous waste were properly trained, based on the following observations: <br /> 1. When asked how employees determine which category of hazardous waste a waste belongs to, Barbaro stated <br /> that the hand held scanner tells employees which hazardous waste category an item belongs in, once an item is <br /> scanned.When Barbaro scanned several hazardous waste items, including but not limited to the hazardous waste <br /> listed below, the scanner did not state which hazardous waste category the waste belonged to: <br /> -An open package of Salonpas Lidocaine Plus Pain Relieving Cream was found in a Ignitable Hazardous Waste <br /> container.When Barbaro scanned the cream's barcode,the scanner stated "Open Consumable Donation Box <br /> 922125493525 Exists. Can More Items Be Added To This Box?Y/N." <br /> -A partially open package of Walgreens Heat Therapy Patches was found in a Toxic Hazardous Waste container. <br /> When Barbaro scanned the patches barcode,the scanner stated "Donate Scanned Item. New Consumable <br /> Donation Box Must Be Created..." <br /> -A partially open and damaged package of Walgreens Benzocaine Oral Pain Relief paste was found in a Toxic <br /> Hazardous Waste container.When Barbaro scanned the paste,the scanner stated"Open Non-Consumable <br /> Donation Box 922125493525 Exists..." <br /> -An open package of Duo Quick-Set Striplash Adhesive was found in a Toxic Hazardous Waste container.When <br /> Barbaro scanned the adhesive, the scanner stated "Donate Scanned Item. New Consumable Donation Box Must <br /> Be Created..." <br /> Barbaro said that per the scanner,the hazardous waste listed above could be donated, but since the items were <br /> likely customer returns since the packages were open,the items were managed as hazardous waste. Barbaro <br /> stated that employees have access to a job aid which informs them of the appropriate hazardous waste category for <br /> waste items.The job aid was not immediately available and Barbaro stated that the job aid was posted on a fixture <br /> near the hazardous waste storage area, but was misplaced. Barbaro requested an employee to print a copy of the <br /> job aid, however once printed,the words on the job aid were too small to read. Barbaro found the job aid used for <br /> all stores except California on the hand held scanner, and he stated that this was the job aid that employees use to <br /> determine which hazardous waste category an item belongs to. <br /> 2.According to Barbaro, the items with open packages, or marked as a customer return or"1506", listed below, <br /> were incorrectly placed in bins labeled for Return Center(RGM), Call-In, and Recall items, and should have been <br /> placed in the 1506 item bin. Return Center(RGM), Call-In, and Recall items are sent to the Walgreens distribution <br /> center in Woodland, California, according to Barbaro.The 1506 item bin is designated for non-saleable items, <br /> damaged items, returned items that cannot be re-sold, items with no bar codes/UPC, and expired items, as stated <br /> on the 1506 item bin label. Based on my observations, conversation with Barbaro, and the information presented <br /> on the 4/27/20 Walgreens document providing the process for handling and disposing unsaleable items, 1506 <br /> items appear to be hazardous wastes waiting to be removed from the facility product inventory and given a <br /> hazardous waste category(e.g.Toxic, Ignitable, Reactive, etc). <br /> -Ketostix Reagent Strips for Urinalysis <br /> -Walgreens Ketone Test Strips <br /> -Aim gel toothpaste <br /> -Enfamil infant formula <br /> -Azo Maximum Strength Urinary Pain Relief <br /> FA0021446 PR0538446 SCO01 06/17/2020 <br /> EHD 22-01 Rev.9/20/2019 Page 5 of 10 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
