S0 JOA U 1 Environmental Health Department <br /> U ; i y I AMENDED <br /> 6/18/2020 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Walgreens#12549 2040 E Mariposa Rd, Stockton June 17, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> Also provide a corrective action statement and a copy of correction letter to the San Joaquin County Environmental <br /> Health Department within 30 days. <br /> This is a Class II violation. <br /> 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> OBSERVATION: <br /> 1. Two boxes of Walgreens Nicotine Mini Lozenge expired April 2020,two boxes of expired Walgreens Nicotine <br /> Lozenge expired May 2020, and two boxes of expired Walgreens Coated Nicotine Gum expired April 2020 were <br /> found in an open cardboard box above the hazardous waste storage area.According to Barbaro Ruan,the store <br /> manager,the expired nicotine items should have been placed in the bin designated for 1506 items,which are <br /> non-saleable items, damaged items, returned items that cannot be re-sold, items with no bar codes/UPC, and <br /> expired items, as stated on the 1506 item bin label.According to Barbaro, 1506 items are scanned and managed <br /> according to the scanner instructions on a daily basis. When Barbaro scanned the expired nicotine waste, the <br /> scanner stated the patches must be disposed of using the Toxic(hazardous waste category). <br /> 2.A package of Walgreens Nicotine Patches expired April 2019 was found marked"RGM"and in an open bin <br /> labeled for Return Center(RGM), Call-In, and Recall items. When Barbaro scanned the nicotine patches,the <br /> scanner stated"Return Center Return Goods"and"Not on RC File." Barbaro stated that the words"Not on RC <br /> File" informs the employee that the nicotine patches cannot be sent back to the Return Center, and thus should be <br /> managed as hazardous waste. <br /> 3.An open cardboard box containing used printer ribbon rolls was observed in the photo department. Barbaro <br /> stated that the used ribbon rolls are to be managed and labeled as corrosive base hazardous waste.According to a <br /> document,found in a binder containing uniform hazardous waste manifests, used photo ribbon rolls are to be <br /> managed and labeled as toxic hazardous waste. <br /> REGULATION GUIDANCE:A container holding hazardous waste shall always be closed during transfer and <br /> storage, except when it is necessary to add or remove waste. <br /> CORRECTIVE ACTION: Immediately close these containers and ensure all hazardous waste containers are <br /> closed when not adding or removing waste.
