The following is an itemized list of underground storage tank violations that have not <br /> been addressed for 99 SHELL* as of April 27, 2020. <br /> Open violations from August 29, 2019 inspection <br /> Violation #209 -Failure to conduct the designated UST operator visual inspection at least once every 30 <br /> days. <br /> 1)A designated operator inspection was performed on February 18, 2019 and the next inspection was performed on <br /> March 21, 2019, 31 days later. The designated operator visual inspections shall be conducted within 30 days or <br /> less of the previous inspection. Ensure that all designated operator inspections are performed within the required <br /> time frames. <br /> 2) The designated operator failed to check and note all required items on the visual inspection reports. <br /> - Section V of the inspection reports were missing follow up actions about scheduling Overfill Prevention Equipment <br /> Testing. <br /> -Section VI of the report was missing the date, name, and signature of the UST owner/operator. <br /> -The designated operator documented visual inspections of UDCs, but left the question regarding UDC sensor <br /> locations blank. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and shall check all required items on the report. Ensure that designated <br /> operators performing visual inspections at this facility are including all of the required information on the reports. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #314-Failed to have secondary containment or secondary containment not tight. <br /> The 87 and diesel fill sumps do not have VPH components, including a water tight shield lid, shroud boots, and <br /> clamps. Every underground storage tank installed on or after July 1, 204 shall have secondary containment <br /> constructed, operated, and maintained to prevent water intrusion and to be impervious to the liquid and vapor of the <br /> substance that is contained. Immediately contact a properly licensed, trained, and certified contractor to make all <br /> necessary repairs and upgrades to bring this UST system into compliance, under permit and inspection of the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #320 -Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing was conducted on April 11, 2019.
