The incorrect and missing information included the following from the inspection reports: <br /> -THe DO reports of 8/25/19 and 4/10/19, The owner operator failed to provide a name, signature and date within <br /> 48hos of receiving the DO report under the acknowledgment of compliance issues <br /> -The DO reports of 12/18/19, 3/13/19 and 2/13/19 did not include the name of owner/operator. <br /> -The DO reports of 12/18/19, 11/20/19, 10/23/19, 8/25/19, 9/25/19, 7/29/19, 6/5/19, 5/8/10 4/10/19, 3/13/19 and <br /> 2/13/19 failed to add a description to all components marked/A or N from section VII through XI <br /> XI <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the Diesel STP Sump, and UDC 7/8 SUMP . <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: The service technician removed approximately halve a gallon of liquid from the diesel <br /> STP sump and UDC 7/8 sump . Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This is a minor violation. <br /> 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> OBSERVATION: The diesel mechanical flappers used as the overfill prevention system were marked as failures on <br /> the 10/11/18 Overfill Prevention Equipment Inspection Report. <br /> REGULATION GUIDANCE: All UST systems must be equipped with an approved, functional overfill prevention <br /> device at all times during delivery operations. <br /> CORRECTIVE ACTION: Immediately discontinue deposition of petroleum into this tank until the component is <br /> repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br /> If the failed component can't be replaced immediately, there is a possibility that the diesel UST system may be red <br /> tagged to prevent fuel inputs. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> FA0000421 PR0231876 SCO01 01/13/2020 <br /> EHD 23-01 Rev.9/20/2019 Page 6 of 7 Underground Storage Tank Program OIR <br /> 1868 E.
