12 5 MEDICAL EMERGENCY RESPONSE PLAN <br /> At least one Dames & Moore employee on site will hold a current certificate in American <br /> Red Cross Standard First Aid This training provides six and one-half hours of Adult <br /> CPR and Basic First Aid If a medical emergency exists, consult the emergency phone <br /> number list and request an ambulance immediately Perforin First Aid/CPR as necessary, <br /> stabilize the injured, decontaminate if necessary, and extricate o. n1Y if the environment <br /> they are in is dangerous or unsafe and ONLY if the rescuers are appropriately protected <br /> for potential hazards they may encounter during the rescue When emergency services <br /> personnel arrive, communicate all first aid activities that have occurred Transfer <br /> responsibility for care of the injured/ill to the emergency services personnel <br /> The following items and emergency response equipment will be located within easy <br />' access at all times <br /> • First Aid Kit and Infection Control Kit, <br /> Eyewash—A 15 minute eyewash (required if corrosives are present) or an appropriate <br /> amount of portable sterile eyewash bottles will be available on site for flushing <br /> foreign particles or contaminants out of eyes The SSO will demonstrate the proper <br /> operation of the umt(s)prior to the start of work, <br /> • Emergency Phone Numbers List, and <br /> • Portable radios for emergency communications in remote areas <br /> Drugs, inhalants, or medications shall not be included in the First Aid Kit <br /> Supplies should be re-ordered as they are used A monthly inventory must be done on the <br /> first aid kit and infection control kit contents and supplies re-ordered that have been used <br /> and not reported <br />' 12 6 INCIDENT REPORT <br /> ALL site injuries and illnesses must be reported to the SSO and PM immediately <br /> following first-aid treatment The SSO will notify the Duty Safety Officer (800-364- <br /> 3765) Work is to be stopped until the PM or SSO and DSO have determined the cause <br /> of the incident and have taken the appropriate action to prevent a reoccurrence Any <br /> injury or illness, regardless of severity, is to be reported to the DHSM on the accident <br /> report form (see Attachment I) <br /> 12 7 OPERATION SHUTDOWN <br />' Under certain extreme hazardous situations the SSO or SSR may request that site <br /> operations be temporarily suspended while the underlying hazard is corrected or <br /> controlled During operation shutdown, all personnel will be required to stand upwind to <br /> AMFOREST 2 1 7 37 <br />
