10 <br /> 13.0 TRAINING, MEDICAL SURVEILLANCE, SITE INSPECTIONS <br /> ' <br /> 13 1 TRAINING AND MEDICAL SURVEILLANCE <br /> All Dames & Moore site personnel will have met the requirements of 29 CFR <br /> 1910 120(e), including <br /> ' • Forty hours of initial off-site training or its recognized equivalent <br /> • Eight hours of annual refresher training for all personnel (as required), <br />' • Eight hours of supervisor training for personnel serving as Site Safety Officers <br /> • Three days of work activity under the supervision of a trained and experienced <br /> supervisor <br /> All Dames & Moore site personnel are participating in medical surveillance programs <br /> that meet the requirements of 8 CCR 5192(f) Current copies of training certificates and <br />' statements of medical program participation for all D&M personnel are maintained by the <br /> Health Systems Group in Denver <br /> In addition, all Dames & Moore site personnel will review this HSP and sign a copy of <br /> the Safety Plan Compliance Agreement, which is found in Attachment C The PM will <br /> maintain these agreements at the site, and place them in the project file at the conclusion <br /> Mof the operation <br /> Prior to the start of operations at the site, the SSO will conduct a site safety briefing, <br /> which will include all personnel involved in site operations At this meeting, the SSO will <br /> discuss <br />' • Contents of this HSP <br /> • Types of hazards at the site and means for minimizing exposure to them <br />' • The type of monitoring that will be performed <br /> • Action levels for upgrade and downgrade of personal protective equipment <br /> • Personal protective equipment that will be used <br /> • Dames & Moore's Injury and Illness Prevention Program <br /> • Site-specific respiratory protection requirements <br /> • Decontamination protocol <br />' • Site control measures, including safe operating practices and communication <br /> • Location and use of emergency equipment <br /> • Evacuation signals and procedures <br />' All site personnel, including subcontractor personnel, are to attend the briefings and sign <br /> b'n <br /> the briefing form <br /> AMFOResT 21 7 39 <br />
