CLOSURE REQUEST REPORT Section No 4 0 Former Amencan Forest Products Company 07/31/01 2801 West Lane Page 4-1 Stockton,California 16 4.0 RWQCB APPENDIX B CHECKLIST Section 4 0 presents the completed closure request checklist from the Tri-Regional Board Staff Recommendations for Preliminary Investigation and Evaluation of Underground Tank Sites Appendix B This section is organized into sixteen subsections presented in the same order as the sixteen items in the request, the request item is in bold type and a response is provided below the item 1. Distance to production wells for municipal, domestic, agriculture, industry and other uses within 2,000 feet of the site.
