OBSERVATION: An inspection was last done on 9/23/2019 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 10/23/2019. This information was submitted late on 7/31/2020, more than 9 months later. REGULATION GUIDANCE: An operator that receives an inspection report shall have 30 days to implement the corrections specified in the inspection report and comply with permit conditions. The corrective action shall include all of the recommendations made in the report. The operator shall submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is submitted and all corrections implemented within 30 days of receiving an inspection report. This is a Class II violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). OBSERVATION: On the Facility Employee Training Certificate dated 8/26/2020, the "initial training dates" listed for the employees were after the "date of assuming responsibility as a facility employee." The initial training dates are not accurate and are the same as the date of training; several of the employees are listed as having received training on previous training logs. REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. OBSERVATION: An approved copy of the monitoring plans were not available for inspection. REGULATION GUIDANCE: A copy of these plans shall be accessible on site at all times. CORRECTIVE ACTION: Immediately locate and retain a copy on site. This is a repeat violation, Class II.
