The incorrect and missing information included the following from the inspection reports: <br /> -On the 7/17/2019 DO report, the designated operator failed to document all the alarms from the attached alarm <br /> history. The missing alarm includes: L 1 sensor out alarm disp 1-2, L2 sensor out alarm disp 3-4, L3 sensor out <br /> alarm disp 5-6, and L4 sensor out alarm disp 7-8, alarms occurred on June 11, 2019 <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the UDC 5/6 SUMP. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: The service technician removed less than one quart of liquid from the UDC 5/6 sump. <br /> Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> The monitoring system certification was conducted on 6/11/2020 and the paperwork inspection completed today <br /> (6/23/2020).An inspection checklist was provided to the facility operator on the day of inspection. The EHD has <br /> written the complete report which replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 07/23/2020. . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
