No waste analysis records were available for <br /> review for the asbestos waste for invoices 3055469(9/6/2017), 1387267(8/23/2017), and 2843413(8/28/2017). <br /> The asbestos wastes were disposed of as non-hazardous waste. A generator shall make a hazardous waste <br /> determination and keep a record of any test results, waste analyses, or other determinations made in accordance <br /> with hazardous waste regulations for at least three years from the date that the waste was last disposed of. <br /> Immediately locate a copy of the waste analysis conducted for the asbestos waste manifests listed above and <br /> submit copies to the EHD. <br /> This is a minor violation. <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> -In the auto shop, one black 55 gallon steel drum containing used oil was observed with a hazardous waste label <br /> missing the physical property, hazardous property, and the words"hazardous waste." <br /> -In room F6(Biology/Chemistry), one black 5 gallon bucket containing Carolina Solution (formaldehyde waste)was <br /> observed with a hazardous waste label missing the address of the generator, the hazardous property, and the <br /> physical property. The contents were also incorrectly,labeled"iodine"and had an incorrect accumulation start date. <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 30, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate($152).
