S A N J D AR I N Environmental Health Department <br /> u j ;"� i Y AMENDED <br /> 10/25/2019 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> LODI MEMORIAL CEMETERY 5750 E PINE ST, LODI October 04, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 608 CCR 66266.130 Failed to properly manage used oil and fuel filters. <br /> OBSERVATION: This facility generates drained used metal oil filters through the servicing of the tractor and <br /> backhoe. Servicing is done onsite at 5750 E Pine St., Lodi by Pape Machinery.The waste generated including used <br /> oil and drained used metal filters is taken to facility address is 8621 S. EI Dorado St., in French Camp by Pape.A <br /> complete three year bill of lading record for used oil filter recycling was not observed during the inspection for the <br /> following date range: October 04, 2016-October 04, 2019. <br /> REGULATION GUIDANCE: <br /> Persons generating,transporting, or receiving used metal oil filters shall use a bill of lading to record the transfer of <br /> used oil filters. Bills of lading must indicate generator,transporter, and receiving company names, addresses, <br /> telephone numbers,the quantity and size of used oil filter containers transferred, and the date of transfer.A copy of <br /> each bill of lading must be kept on the premises of the generator,transporter, and receiving facility where the used <br /> oil filters were handled. Copies of bills of lading shall be kept for a period of three years. <br /> CORRECTIVE ACTION: Immediately obtain all missing bill of lading recycling records that can be found. Used <br /> metal oil filters not stored, handled and disposed of according to this section are regulated as hazardous waste. <br /> Provide a corrective action statement, bill of lading recycling records and supporting documentation to the San <br /> Joaquin County Environmental Health Department(EHD)within 30 days. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by November 24, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152). To schedule an <br /> appointment, please call (209)468-3420. <br /> Documents provided during inspection: Hazardous Waste Emergency Information form. <br /> Waste streams found: Oily solids: shop rags, used absorbent, two empty 55 gallon drums that once contained <br /> hazardous waste <br /> Note: Cemetery Superintendent stated facility is outsourcing their maintenance operations that once generated <br /> hazardous waste. <br /> Pape, services the tractor and backhoe at the cemetery and takes waste generated such as used oil and <br /> drained used metal oil filters to Pape Machinery facility at 8621 S. EI Dorado St., in French Camp. <br /> Odyssey Landscaping, Inc, 5400 CA-12, in Lodi performs ground maintenance. <br /> Cemetery Cars (golf carts)are serviced off site at Acme Saw and Supply, 214 N.
