This notification has <br /> never been made to PHS-EHD. A site visit will be performed by PHS-EHD staff <br /> during the next sampling activity and during most other sampling events. Also, <br /> PHS-EHD staff prefers to be on site to observe the start up of the soil vapor <br /> extraction system. Please provide notification to PHS-EHD by calling Linda <br /> Turkatte at the number shown below at least two business days in advance <br /> so that PHS-EHD staff can be on site for these activities. <br /> Several dates in the report were reported incorrectly in the tables and in the <br /> laboratory results. The illegibility of the sampling sheets and chain of custody <br /> may be partially responsible for these errors. However, as these reports should <br /> be as accurate as possible and not be misleading or inconsistent in content, <br /> future reports should be given more attention before being submitted. <br /> Please add methyl tertiary butyl ether (MTBE) to the constituents of <br /> concern for this site and analyze for this compound effective immediately. <br /> A Division of San]oagWn County Health Care Services <br />
