Wilson Way I <br /> Stockton, Ca. 95205 If Site Code: 1299 <br /> San Joaquin County Public Health Services, environmental Health Division <br /> s (PHS/EHD) has reviewed the "Workplan for Additional� Site Assessment" for the <br /> above mentioned site. This letter serves to provide approval for this workplan <br /> as written. Please provide another copy of thislworkplan„yto PHS/EHD along with <br /> a $267 check and 3 permit applications. The City,10f Stockton should be contacted <br /> at (209) 944-8366 to obtain encroachment permits for Harding Way and Wilson Way . <br /> through their Public Works Department. <br /> .i <br /> PHS/EHD has reviewed the Quarterly Monitoring Report dated Feb. 24, 1994 for the <br /> above mentioned site. The last sampling reported in �a Quarterly Report to this <br /> J office occurred March 31, 1993. The report dated Feb) 24, 1994 contains results <br /> from three samplings: (6/23/93, 9/13/93, and 12/15/93) . <br /> 'E <br /> Pursuant to Section 2652(4) of the Underground .Storage Tank Regulations <br /> (California Code of Regulations, Title 23, Chapter 16) a quarterly monitoring <br /> report shall be submitted to Public Health Services, Environmental Health <br /> Division and Central Valley Regional Water Quality Control Board (CVRWQCB) until <br /> the investigation and cleanup of the site is deemed adequate. The report is to <br /> show the: I <br /> -status of investigation and cleanup activities including the results of � <br /> all investigations completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the completion <br /> of the investigation of the site and remediation; <br /> -method and location of disposal of the released hazardous substance and <br /> any contaminated soil, groundwater or surface mater; <br /> -manifest required for transport of hazardous substances; <br /> -monitoring well data including groundwater elevation, pH, conductivity, <br /> gradient; f <br /> -analytical results for groundwater or soil sampling; and j <br /> -site map showing the "zero line" of contaatinat`ion, if known, and changes <br /> in the above measurements overthe Last quarter. 11 <br /> Liquid phase hydrocarbons have been reported in MW-5 on 6/23/93, 9/13/93, and <br /> 12/15/93. The source of this hydrocarbon contamination has yet to be identified. <br /> The source should be identified and corrected. !M <br /> According to Section 2652 (e) of the Calif ornia !lUnderground Storage Tank <br /> Regulations, free product removal reports prepared in compliance with Section <br /> 2655 of this article shall be submitted to the lfocalilagency (PHS/EHD) within 45 1 <br /> calendar days of release confirmation.
