Well boxes and well heads were in disrepair, <br /> Quarterly Monitoring reports were late or missing and incomplete,corrective action plan requirements(air pollution <br /> permits,schedules, system on-line,etc)were not met,and a well head survey had not been conducted <br /> PHS-EHD advised Thrifty Oil Company(TOC)that if the above noted concerns were not adequately addressed,PHS- <br /> EHD would respond to the SWRCB-CUF Compliance Review Form and recommend "Claimant not in corrective <br /> action compliance-rejection recommended". Although no date was set for all the work to be completed,it was <br /> understood in conversations between Michael Infurna of PHS-EHD and yourself over the past few months that the <br /> summer of 1999 would be more than adequate time to ensure that TOC met all of PHS-EHD's requirements. <br /> On April 28, 1999 and again on April 30, 1999,PHS-EHD conducted inspections for well head and traffic box repairs. <br /> Although much of the needed work was done,not all of the wells were repaired properly. Corrections were made and <br /> on May 4, 1999,another field inspection conducted by PHS-EHD revealed that all of the necessary well head and traffic <br /> box repairs were completed. Thank you for your field technician's devoted attention to detail and good work. <br /> In order to complete the SWRCB-CUF review form,PHS-EHD will evaluate this site for compliance on July 1, 1999. If <br /> all the above items have been adequately addressed and the Soil Vapor Extraction(SVE) system is onsite and operating <br /> as it was designed,PHS-EHD will indicate that the site is in compliance.
