The following is an itemized list of hazardous materials violations that have not been addressed for Tire & Wheel Outlet as of July20, 2020. Open violations from June 18, 2020 inspection Violation#2- Failed to complete and electronically submit a hazardous materials business plan. The facility's business plan was not submitted electronically to the California Environmental Reporting System (CERS)for 2018, 2019, and 2020. As of January 1, 2013, all businesses were required to submit all business plans electronically. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the corrector updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#3-Failed to establish and implement a hazardous materials business plan. A Hazardous Materials Business Plan has not been established or implemented for 2018, 2019, and 2020. A Hazardous Materials Business Plan shall be established and implemented for emergency response to a release or threatened release of a hazardous material when storing hazardous materials at or above the thresholds quantities of 55 gallons for materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and pressure. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the required information, and submit for review by the EHD. ❑ This violation was corrected ❑ This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#4- Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. The Business Activities and Business Owner/Operator Identification forms have not been submitted for 2018, 2019, and 2020. This information must be current and submitted as part of the business plan.
