The following is an itemized list of hazardous materials violations that have not been <br /> addressed for Tire & Wheel Outlet as of July20, 2020. <br /> Open violations from June 18, 2020 inspection <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#8- Failed to submit a training program or program is not reasonable or adequate for facility. <br /> A training program has not been submitted 2018, 2019, and 2020. Training programs for all employees and annual <br /> training, including refresher courses, shall include familiarity with the facility's Emergency response plans and <br /> procedures in the event of a release or threatened release of a hazardous material. Training shall cover, but not <br /> limited to, all of the following: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. <br /> These training programs may take into consideration of each employee. Immediately prepare or update the facility's <br /> training program, log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually for 2018, 2019, and 2020. The hazardous materials inventory shall be submitted by <br /> January 15 of each calendar year and may be submitted beginning November 1 of the previous year.
