SAN J 0 A Q U I N Environmental Health Department <br /> C a u N T Y Irl AMENDED <br /> 6/22/2020 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> PAQ Inc DBA FOOD 4 LESS 3434 MANTHEY RD, STOCKTON June 19, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: The business operator could not provide hazardous waste disposal records for the 8 ounces of <br /> alcohol waste generated on 10/4/2018 and 22 ounces of Best Yet Lighter Fluid waste generated on 1/12/2019, <br /> according to the Flammable Hazardous Waste Log posted in the hazardous waste storage area. <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in <br /> accordance with section 66262.23(a)for three years or until the generator receives a signed copy from the <br /> designated facility which received the waste.This signed copy shall be retained as a record for at least three years <br /> from the date the waste was accepted by the initial transporter. <br /> CORRECTIVE ACTION: Provide a copy of disposal records for the 8 ounces of alcohol hazardous waste <br /> generated on 10/4/2018 and 22 ounces of Best Yet Lighter Fluid hazardous waste generated on 1/12/2019. <br /> Provide these records and a corrective action statement to the EHD within 30 days. <br /> This is a Class II violation. <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION: The emergency eye wash station in the facility's back storage area did not function when tested. <br /> REGULATION GUIDANCE:All facility communications or alarm systems,fire protection equipment, spill control <br /> equipment, and decontamination equipment,where required, shall be tested and maintained as necessary to <br /> assure its proper operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the emergency eye wash station into proper working order by repair, replacement or <br /> testing.
