The incorrect and missing information included the following from the inspection reports: <br /> -The DO reports of 7/9/19, 8/8/19, 9/5/19, 10/2/19, 10/3119, 11/27/19, 1/24/20, 2/19/20, 3/16/20, and 4/15/20. The <br /> DO failed to add a description in section III to all components marked NA from section VII through XI. <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a minor violation. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> OBSERVATION: Small amounts of liquid was observed in UDC#3/4 indicicating a leak in the primary containment. <br /> REGULATION GUIDANCE: All primary containment for the UST system must be product tight. <br /> CORRECTIVE ACTION: Immediately have a properly licensed, trained, and certified contractor repair or replace the <br /> failed component(s), under permit and inspection of the EHD if necessary. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the 87 and diesel STP sump. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: The service technician removed approximately halve a gallon of liquid from the 87 and <br /> diesel STP sump. Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by June 4, 2020 . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. <br /> FA0003607 PR0232388 SCO01 05/04/2020 <br /> EHD 23-01 Rev.9/20/2019 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
