The following is an itemized list of underground storage tank violations that have not been addressed for GRANT LINE SHELL* as of May 12, 2020. Open violations from May 30, 2019 inspection Violation#103 -Failure to submit and maintain complete and current financial responsibility documents. Financial responsibility documents have been submitted to the EHD but are not found on site. Current financial responsibility documents are required to be submitted annually. Ensure a copy of the financial responsibility records are found on site. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#109 -Failed to test secondary containment, or failed to maintain test records for 36 months. Penetrattions were replaced in all product, in all UDCS, in all STP sumps and were tested on 5/24/18, 7/5/18, 7/6/18, and 7/12/18 and six month testing was due by november 2018, and January 2019. Testing has not been completed and is 4 months past due. Secondary containment testing is required six months after installation and at least every 36 months after. Immediately schedule this test and provide 48 hours notification to the EHD. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#110- Failed to submit"Secondary Containment Testing Report Form"within 30 days after the test. Secondary containment testing was performed on 5/24/18, 7/5/18, 7/6/18 and 7/12/18. The test report were submitted to EHD on 9/28/18, over three months late for the test conducted in May 2018 and over a month late for the test conducted in July 2018. A copy of the test report must be submitted within 30 days of the testing.
