An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and the <br /> following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. During the inspection Gayle Moses <br /> completed the modified contingency plan and had it posted by the facility phone. Corrected on site. <br /> 808. The universal waste(UW) electronics were not labeled to identify type of UK A UW handler shall <br /> label or mark UW to identify the type of waste. Electronics (i.e., each electronic), or a container in which <br /> the electronic(s) are contained, shall be labeled or marked clearly with the following phrase: "Universal <br /> Waste-Electronic(s)". During the inspection Gayle Moses completed the UW label by adding the word <br /> Electronic after the words universal waste.
