Broadway, Stockton <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS 11, or MINOR -Notice to Comply) <br />63. Copies of hazardous waste disposal records for first two quarters of 2010, and entire year of 2009 <br />were not found on site. <br />-- Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and <br />all receipts used in a consolidated manifesting procedure on site for three years and have them readily <br />available for review. Immediately locate a copy of all manifests and receipts for the last three years, <br />maintain them on site, and submit copies to the EHD by July 25, 2011. <br />79,83,84. Unable to determine the length of storage of about 23 Universal Waste fluorescent lamps not <br />stored in a container, and one broken lamp not containerized. <br />-- Broken Universal Waste lamps shall be immediately cleaned up and containerized to prevent release <br />to environment, and make a hazardous waste determination and manage it accordingly. Universal <br />Waste lamp (unbroken ones) shall be stored in a closed container that prevents breakage and release to <br />the environment, labeled to identify the type of Universal Waste and the initial date of accumulation, <br />managed within one year of storage, and retained a copy of the disposal receipt for at least 3 years on <br />site. <br />* Immediately dispose of the above Universal Waste - lamps and submit a copy of the disposal <br />receipt to EHD by July 25, 2011. <br />NOTES <br />The facility generated 5217 pounds of hazardous waste in 2010. Facility retails agricultural chemicals <br />and fertilizers and generates related wastes, and waste from equipment/vehicle maintenance. <br />Discussed with generator Universal Waste management requirements including aerosol cans, lamps, <br />alkaline batteries, mercury containing switches, CRT, electronic devices, etc. in a closed container with a <br />label identifying the content and the start date of accumulation. <br />A copy of CUPA class schedule, blank hazardous waste labels, hazardous waste labeling fact sheet, <br />and San Joaquin County Small Business Hazardous Waste Disposal Program pamphlet, have been <br />provided to generator during this inspection. <br />Submit the following to EHD by: July 25, 2011 <br />1) A completed copy of the (provided) Return to Compliance Certification (please keep original for <br />your records). <br />2) A written statement how each violation was corrected <br />3) Evidence (i.e. photos, disposal receipts, documents) in support of the corrections made. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EM,r- <br />Recei� �ByQ: <br />Title: <br />EHD 23-02-003 Rev 08/10/10 J CONTINUATION FORM <br />
