Main St., Stockton <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 11, or MINOR -Notice to Comply) <br />This is a revised version of the original HW inspection report generated on <br />December 28, 2010 The version supersedes all previous versions. <br />23. No label on one open 55 -gallon drum of used oil filters. Drained used oil filters shall be <br />accumulated, stored, and transferred in a close rainproof container that is properly labeled with the <br />words "drained used oil filters" and the initial date of accumulation, and managed within one year of <br />storage. A copy of the used oil filters disposal receipts shall be retained on site for at least 3 years <br />available for inspection. Immediately close the above container, dispose the drained oil filters in <br />accordance to CCR Title 22, and submit a copy of the disposal receipt to EHD. <br />27. Containers identified in above Item 20 & 50, except the 30 -gallon container of used oil, were not <br />closed. All hazardous waste containers shall be closed at all times except when adding or removing <br />waste. Immediately close these containers and ensure all hazardous waste containers are closed when <br />not adding or removing. <br />52. An emergency coordinator and modified contingency plan information is lacking. There must be at <br />least one emergency coordinator on site or on call to coordinate emergency response measures, and <br />the following information must be posted by a phone: the name and phone number of the emergency <br />coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br />phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br />emergency coordinator and post the required information by a phone. A form is provided that can be <br />used for this purpose. <br />NOTES <br />The facility generated less than 5 tons of HW in 2009. The facility is no longer in business, but has <br />stored hazardous waste on site in 2010. The facility failed to return to compliance as required in <br />December 21, 2009 routine hazardous waste inspection report and the subsequent notification of failure <br />to return compliance status dated October 5, 2010. This is a follow-up inspection billable at current EHD <br />hourly rate. <br />Submit the following to EHD by January 31, 2011: 1) A copy of the completed Return to Compliance <br />(attached), 2) A written statement how each of the above violation was corrected, 3) Evidence (i.e. <br />photos, disposal receipts, documents) in support of the corrections made. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspe r: <br />Received By: <br />Title: <br />%, <br />EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
