Manifest numbers 001078239SKS & 001982970SKS were found without a signed off copy from the destination <br />facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three <br />years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten <br />signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the <br />waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the <br />designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the <br />manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an <br />Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining <br />the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Within 30 days, <br />submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. <br />79. UW lamps storage. At time of inspection: <br />Two 4 -ft used lamps were in a open metal box, and fifteen 4 -ft used lamps were in a open cardboard box. Facilities must <br />store UW lamps in a closed container and in a manner that prevents breakage and releases to the environment and <br />immediately clean and containerize and broken lamps or releases to the environment. Immediately properly store UW lamps. <br />83. Failed to label UW to identify type of waste. At time of inspection: <br />UW lamps were not labeled adequately. Universal containers must be labeled at a minimum, "Universal Waste" and the type <br />of waste being stored. Immediately properly label containers of UW. <br />84. Stored UW on site for more than 1 year. At time of inspection: <br />A bucket of alkaline batteries was dated 1/09. Universal Waste may only be stored on site for up to one year. Immediately <br />have this Universal Waste removed and provide this office with a copy of the shipping record. <br />999. Failure to submit Return to Compliance Certification for Class II & Minor Violations HSC 25187.8(b) & 25185(c)(3): <br />Return to compliance certification was not submitted for the 2008 hazardous waste inspection. The outstanding violations <br />noted on the 2008 report are repeated on this report. Immediately correct all hazardous waste violations and submit the <br />Return to Compliance Certification to this office. <br />Complete and submit a copy of the Return to Compliance Certification with a statement addressing how each of the above <br />listed violations have been corrected to this office within 30 days.
