General Pretreatment Industrial Users <br />Permit Conditions <br />15. Permittee shall maintain all pretreatment records for at least three years. All accidental spills <br />or upsets must be reported to this Department immediately. This shall be accomplished by <br />telephone, and shall be followed by written report within five working days by Permittee to <br />correct the situation and to prevent its recurrence. Failure to report a spill or upset will be <br />grounds for civil or criminal penalties, depending upon circumstances. <br />16. Permittee must meet all applicable regulations under subtitles C and D of the Resource <br />Conservation and Recovery Act, and 40 CFR Parts 260 through 271, and any applicable State <br />regulations promulgated subsequent to the Federal law. <br />17. Permittee shall prepare and submit a spill prevention plan within 30 days from the effective <br />date of this Permit to assure that liquid wastes are contained on site at all times. <br />18. The permittee shall have a water conservation and hazardous waste minimization plan. <br />19. The discharge of excess amounts of pollutants specifically limited and deviations of Ph may <br />be subject to fines for each violation. <br />20. Achieving Permit Conditions of specific pollutant concen-tration limits by dilution is <br />expressly prohibited. Any evidence of this, either willfully or through negligence, will result <br />in Permit Limits being redefined in terms of maximum effluent loadings. Additionally, <br />penalties of a maximum of $6,000 per day may be assessed (City Code 7-091.12). <br />21. Signatory Requirements: All applications, reports, or information submitted to the City of <br />Stockton must contain the following certification statements and be signed as required in <br />Section (a), (b) and (c) below: <br />"I certify under penalty of law that this document and all attachments were <br />prepared under my direction or supervision in accordance with a system <br />designed to assure that qualified personnel properly gather and evaluate the <br />information submitted. Based on my inquiry of the persons directly <br />responsible for gathering the information, the information submitted is, to the <br />best of my knowledge and belief, true, and accurate, and complete. I am <br />aware that there are significant penalties for submitting false information, <br />including the possibility of fine and imprisonment for knowing violations. " <br />1. By a responsible corporate officer, if the Industrial User submitting the reports is a <br />corporation. For the purpose of this paragraph, a responsible corporate officer <br />means: <br />a.
