Fremont St <br /> Stockton CA 95205 40 <br /> The San Joaquin County Environmental Health Department(EHD)has completed the review of <br /> the application for permit(SR0052249) to conduct the following work: <br /> • Repair the 87 STP Sump <br /> • Repair the 91 Fill Sump <br /> 1. Contact the Fire Department and San Joaquin Valley Air Pollution Control District for <br /> applicable permits. <br /> 2. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. All valid, current California UST certificates issued by the International Code <br /> Council (ICC), that are applicable to the work being performed, and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested, repaired, or serviced. <br /> and <br /> d. Each person installing or testing any component must hold a certificate of training <br /> from the manufacturer of that component. <br /> 3. All materials used must be certified by an independent testing organization and <br /> bear the appropriate mark as required by the independent testing organization. <br /> 4. Contact Michelle Henry(953-7699)to arrange the following inspections appointments <br /> (a 48-hour notice is required for inspection appointments): <br /> a. Inspection of 87 STP sump and 91 Fill sump to verify repairs. <br /> 5. Complete and submit a copy of a UST Installation Certificate of Compliance(Former <br /> Form C) for the 87 sump and the 91 sump no later than 30 days after work is <br /> completed. <br /> 6. If concrete is broken above the tank pit or over piping runs, a tank integrity test must <br /> be performed by a third party certified tank tester after concrete is poured. Results <br /> must be submitted to this office within 7 days of completion of work. <br /> 7. This permit expires 90 days from the approval date. <br />
