Two (2) completed UST Application and Worksheets - The contractor information will be <br /> provided at the time of permit issuance <br /> 2. Two (2) UST Service Request Forms <br /> 3. Two (2) UST—Facility forms (Form A) <br /> 4. Six (6) UST Tank forms (Form B), two for each tank <br /> 5. Two (2) sets of tank and piping plans, signed <br /> 6. Two (2) copies of manufacturers'equipment cut sheets <br /> 7. One (1) Barghausen Consulting Engineering, Inc. check made payable to San Joaquin <br /> Environmental Health Department in the amount of$20 <br /> It is our understanding that the items referenced above along with the materials submitted on <br /> June 29, 2010 should comprise a complete package for the new Underground Storage Tank <br /> Application.
