No. TBD/Our Job No. 14573 <br /> Dear Kasey: <br /> On behalf of our client, Costco Wholesale, Barghausen Consulting Engineers, Inc. is submitting for an <br /> Underground Storage Tank Permit for a new fueling facility at the above referenced location referenced. <br /> The facility will be constructed in the southeast corner of the proposed Costco parcel which is part of a <br /> larger retail shopping center development proposal. This project will include the construction of a fully <br /> automated, attended, members-only, self-service retail fueling facility. The fueling facility consists of a <br /> new pump island canopy with 8 multi-product dispensers (16 fueling positions). The project also includes <br /> the installation of three 30,000-gallon underground gasoline storage tanks and a 6- by 12-foot controller <br /> enclosure that will house the electronic monitoring and alarm system. Costco Gasoline facilities typically <br /> operate during the hours of 6 a.m. to 10 p.m., Monday through Friday, and 8 a.m. to 8 p.m. on Saturday <br /> and Sunday. <br /> The following items are enclosed for your review: <br /> 1. One (1) completed UST Application and Worksheet - The contractor information will be <br /> provided at the time of permit issuance <br /> 2. One (1) UST Service Request Form <br /> 3. One (1) UST— Facility form (Form A) <br /> 4. Three (3) UST Tank forms (Form B), one for each tank <br /> 5. Three (3) sets of tank and piping plans, signed <br /> 6. One (1)set of manufacturers'equipment cut sheets <br /> 7.
