The permit has been approved with <br /> the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Veloso at(209)468-3453 to arrange the following appointments (a 48-hour notice is <br /> required for inspection appointments). <br /> • Sensor functionality: Functionality of the monitoring system must be verified to ensure that <br /> monitoring has been restored to meet the requirements of Title 23 California Code of Regulations <br /> and the Health &Safety Code. A copy of the alarm history shall be printed prior to performing <br /> the"cold start" of the monitoring system and made available at the time of EHD inspection. <br /> • Leak detector test: Leak detector must be tested to confirm that the device can detect a leak of 3 <br /> gallons per hour at 10 psi. <br /> • Overfill prevention verification: Verify that external audible/visual alarm activates when tank is 90% <br /> full. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD inspection <br /> and approval. <br /> Please be aware that the facility cannot receive fuel in the tank prior to the EHD inspection <br /> and approval. <br /> 4. If concrete will be broken near the tank, a secondary containment test must be performed on the <br /> tank once concrete is poured in accordance with the manufacturer's guidelines. <br />
