During your visit,you brought a few issues of concern to <br /> our attention.We have noted each issue below with our plan to address your concerns. <br /> concern-#l: <br /> The L1191 fill sump sensor failed to activate an audible and visual alarm when tested to ensure continuous <br /> monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual <br /> alarm or stop the flow of product at the dispenser when it detects a leak. The sensor was replaced and <br /> retested during the inspection. A"UST Retrofit Verification with Inspector Already Onsite"form has been <br /> completed and provided to the operator and contractor. <br /> Response: <br /> _Resolved by All Tech Petroleum who performed the monitor certificotion on 5128114. <br /> Concern#2: <br /> Approximately one cup of water was found in the 91 turbine sump indicating a leak in the secondary <br /> containment. Secondary containment shall be impervious to the liquid and vapor of the substance,contained <br /> and constructed to prevent structural weakening as a result of contact with any hazardous substances released <br /> from the primary containment. Immediately contact a properly licensed trained and certified contractor to <br /> made repairs to the UST system under permit and inspection of the EHD. <br /> Response: <br /> Concern was inspected by braes Covey Group, Inc.on 6113114. See attached work order#30476 <br /> Concern_0: <br /> The S9 87A product VPH smart sensor failed when tested. The interstitial space of the UST system shall be <br /> maintained,as designed, under constant vacuum such that breach in the primary or secondary containment is <br /> detected before the hazardous substance is released into the environment. The sensor was replace and retested <br /> during the inspection.
