The following are Minor-Notice to <br /> Comply violations: # 311, 313 <br /> 311. The mechanical line leak detector could not be tested because the turbine pump, which power has <br /> been disconnected, could not be turned on. All line leak detectors shall be capable of detecting a 3- <br /> gallon per hour leak at 10 psi, and be tested annually. Immediately have a properly licensed, trained, <br /> and certified tester test the above detector under EHD inspection. Notification of testing must be <br /> submitted by fax (468-3433) to EHD at least 2 days in advance. <br /> 313. The direct-bury spill container failed when tested. All spill containers shall have a minimum <br /> capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br /> Immediately discontinue deposition of petroleum into this tank until the component is repaired or <br /> replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br /> If the spill container can't be replaced immediately, there is a possibility that the UST system may be red <br /> tagged to prevent fuel inputs. <br /> NOTES <br /> The technician on site provided the following manufacturer's certifications: <br /> Petro Tite Test (for line test) Expiration 9/3/13 <br /> VMI catastrophic testing: Expriation 11/10/13 <br /> Leak Alert: CN 120588 Expiration 11/7/12 <br /> Observed annual line tightness test: passed <br /> Received copy of latest Financial Responsibility on site policy period from 12/28/2012 to 12/18/2013. <br /> Previous Financial Responsibility document on file covers 2/23/2012 to 2/23/2013. <br /> Designated Operator Monthly Inspection Reports for March, April, May, October,September, November, <br /> December 2012, and January 2013, referenced "suction" on leak detector section on page 2. This is <br /> incorrect.
