Acacia Street <br /> Stockton, CA 95203 <br /> The San Joaquin County Environmental Health Department(EHD) has completed the review of <br /> the application for a permit to install a VMI 99-LD2000 line leak detector on the underground <br /> storage tank (UST) system at the above noted address. The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Jeff Wong at(209) 468-0335 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Leak detector test: Leak detector must be tested to confirm that the device can <br /> detect a leak of 3 gallons per hour at 10 psi. <br /> Please be aware that the facility cannot operate the UST tank prior to the EHD <br /> inspection and approval. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being tested, repaired, or serviced. <br /> 5. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site who possesses all of the following: <br />
