(AGE) has prepared this enclosed Underground <br />Storage Tank Removal Permit Application for 1756 North Wilson Way, Stockton, California. Two 10,000 -gallon and <br />one 6,000 -gallon underground storage tanks (USTs) are located on the property and will be removed. Also enclosed are <br />the following items: <br />1. A detailed site plan; <br />2. Permit application fee of $732.00; <br />San Joaquin County Public Health Services - Environmental Health Division (PHS-EHD) Service Request <br />Form. <br />Additional USTs were previously removed from the property and an investigation is ongoing. A site health and safety <br />plan is presently on -file with the PHS-EHD. Additional copies can be submitted, if necessary. <br />Field procedures will commence upon approval of the Permit Application.
