This is a revised report issued on August 8, 2014 and it supersedes all previously <br /> issued reports. <br /> Class II Violations: 204, 311 <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> August 6, 2013 and September 9, 2013 designated operator monthly inspection reports and failed to <br /> check that they were responded to appropriately. The missing alarms include: July 11, 2013 (1_1 through <br /> L6 alarms), July 15, 2013 (L6 alarm), August 13, 2013 (L4 and L6 alarm). During the monthly <br /> inspection, the designated operator shall review the alarm history for the previous month, check that <br /> each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> CORRECTED. <br /> 204. The designated operator failed to check and note the correct test dates for the 2013 monitoring <br /> system certification on the January 2014 through July 2014 designated operator monthly inspection <br /> reports. During the monthly inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. CORRECTED. <br /> 311. The regular unleaded line leak detector failed to detect a leak when tested. All line leak detectors <br /> shall be capable of detecting a 3-gallon per hour leak at 10 psi. The leak detector was replaced and <br /> retested during the inspection. A "UST Retrofit Verification with Inspector Already Onsite" form has been <br /> completed and provided to the operator and contractor.
