Clay SL, Studdon <br />The San Joaquin County Environmental Health Department (END) has completed the review, of <br />the application for a permit to replace the 67 grade mechanical line leak detedor on the <br />underground storage tank (LAST) system at the above noted address. The permit has been <br />approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Ar Polkrtion Control <br />District Por any applicable permits and requirements. <br />2. Any changes in information and scope of work rmrst be submitted to EHD for approval <br />prior to commencing work All new addenda supersede the previous ones. <br />3. Contact Stacy Rivera at (206) 4663440 to arrange the following inspection appointment <br />(a 48Iwur notice is required for Inspection appointments). Please be aware that <br />Inspections for emergency work must be scheduled within five days of panne approvaL <br />a. Leak detector test: Leak detector must be tested to confirm that the device can <br />dated a leak of 3 gallons per hour et 10 psi. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />fopowing: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license Issued to the individual by the Stege Water Board, and <br />b. A valid, amend Calfomia UST Service Terhnicran certificate issued by the <br />International Code Council (ICC), and <br />c. A valid arrest oadifxate of training trove the manufacturer(s) of the component(s) <br />being tested, repaired, or serviced. <br />
