This work was completed on November 18, 2015. The permit <br /> has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Stacy Rivera at(209) 468-3440 to arrange the following appointments (a 48-hour notice is <br /> required for inspection appointments). Please be aware that inspections for emergency work must be <br /> scheduled within five days of permit approval. <br /> • Leak detector test: Leak detector must be tested to confirm that the device can detect a leak of 3 <br /> gallons per hour at 10 psi. <br /> 4. If concrete will be broken near the tank, a secondary containment test must be performed on the <br /> tank once concrete is poured in accordance with the manufacturer's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, current <br /> contractor's licenses issued by the Contractors State License Board (may be issued to the <br /> installation company), or a Tank Tester's license issued to the individual by the State Water <br /> Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically present at <br /> the work site who possesses a valid, current California UST Service Technician certificate issued <br /> by the International Code Council (ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s)of the component(s) <br /> being tested, repaired, or serviced. <br />
