Annual monitoring system certification, leak detector testing, and spill container testing was performed on July 28, 2014 and a test report was submitted on January 9, 2015, five months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 208 CCR 2715 Designated operator monthly inspection reports not maintained on site for at least 12 months. The designated operator monthly inspection reports were not found on site for every month. -The December 2014 designated operator monthly inspection report is dated 12/3/14, but the attached alarm history is dated 12/10/14. -A designated operator monthly inspection report is dated 12/8/15, and the attached alarm history is dated 1/8/15. -A designated operator monthly inspection report is dated 12/8/15, and the attached alarm history is dated 3/4/15. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD. Note: The April 2015 designated operator monthly inspection report is dated 4/1/15, but the attached alarm history is dated 3/31/15. On 4/21/15 a note was made in the repair log that the monitoring panel time was incorrect and had to be adjusted. According to EHD records and the paperwork on site, EHD was on site for testing of the diesel STP sump sensor on 4/15/15, however, the alarm history from the monitoring panel listed this testing as occurring on 4/14/15. According to Mr. John Bertao, the April alarm history was printed on 4/1/15 before he noticed the incorrect monitoring panel date. This is a minor violation. 209 CCR 2715(f) Designated operator employee training not performed or log not kept. The designated operator employee training records were incomplete. They did not include a date of hire for each employee. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms A list of facility employees who have been trained by the designated UST operator(s), shall be maintained on-site or off-site at a readily available location, if approved by the local agency. The list shall be provided to the local agency upon request. The list shall include the dates of training for all facility employees, and the hiring dates for all facility employees hired on or after July 1, 2005. Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a repeat violation, Class II.
