Locate and <br /> ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on <br /> site. Submit copies to the EHD. <br /> Note: The April 2015 designated operator monthly inspection report is dated 4/1/15, but the attached alarm history is <br /> dated 3/31/15. On 4/21/15 a note was made in the repair log that the monitoring panel time was incorrect and had to <br /> be adjusted. According to EHD records and the paperwork on site, EHD was on site for testing of the diesel STP <br /> sump sensor on 4/15/15, however, the alarm history from the monitoring panel listed this testing as occurring on <br /> 4/14/15. According to Mr. John Bertao, the April alarm history was printed on 4/1/15 before he noticed the incorrect <br /> monitoring panel date. <br /> This is a minor violation. <br /> 209 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training records were incomplete. They did not include a date of hire for each <br /> employee. The designated operator shall train facility employees for which he or she is responsible in the proper <br /> operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> A list of facility employees who have been trained by the designated UST operator(s), shall be maintained on-site or <br /> off-site at a readily available location, if approved by the local agency. The list shall be provided to the local agency <br /> upon request. The list shall include the dates of training for all facility employees, and the hiring dates for all facility <br /> employees hired on or after July 1, 2005. Ensure that employees have been trained by the designated operator, <br /> maintain the list on site, and submit a copy of the completed training records to the EHD. <br /> This is a minor violation. <br /> 323 CCR 2636(%2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 87 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting <br /> a 3-gallon per hour leak at 10 psi.
