Alpha <br />Petroleum' <br />Services, Inc. <br />General Engineering A Hazardous C61/D64 C10 No 742466 <br />Maintenance—Construction—Electrical Contractor <br />ADDITIONAL SCOPE OF WORK <br />For <br />USA Gasoline #835, 2705 Country Club Blvd, Stockton, CA <br />Since we have to expose the fill and vapor risers to the top of the UST's we felt it would be practical to <br />install sumps and Phil-Tite manholes and EVR Buckets to meet the upcoming state EVR regulations for <br />2005. So we are asking to include within our permit the following Scope of Work: <br />• Once we are down to tank top we will remove the existing risers for the fill and vapor buckets. <br />• The existing Bung holes measures 24" in-between the fill and vapor risers. <br />• We will install a Western Fiberglass Sump with a bottom to using tank bung adaptors as per "Cut <br />Sheet 1 ". <br />• We then install a Phil-Tite type offset to make up the difference between being 24" centers from fill <br />and vapor bung holes to the Phil-Tite 16" SC -3600 -TSC -2-1 Man -way lids. Off sets "Cut Sheet 2 <br />and 3", Phil-Tite lids "Cut Sheet 10" <br />• The Extractor Vent Valve we will be installing is a ERW and is shown in "Cut Sheet 4" <br />• We will install 4" risers steel riser pipes for the fill and vapor in one length, not with a coupler <br />• We will install conduit penetrations and run conduit to install sump sensors <br />• The Torque Tool Adapter we will be using is shown in "Cut Sheet 5" <br />• We will use Phil -T SC -3600 -TSC -2-1 Type fill and vapor buckets as per attached "Cut Sheet 6" <br />We will backfill sumps with pea gravel <br />• The drop tubes will be OPW 61 T shown in "Cut Sheet 7" <br />• Once all penetrations are complete and Risers installed we will test sump for tightness. <br />• We will use Western Fiberglass Type B Reducer Lids as per "Cut Sheet 8" <br />• For our Watertite Sump Shield we will us the Phil-Tite Type FG-3433-WTSS as per "Cut Sheet 9" <br />• Then our Manhole cover will be Phil-Tite SC -3600 -TSC -2-1 as per "Cut Sheet 10" <br />• When complete we will call for inspection before we pour concrete <br />• Once we have inspection we will place re -bar and pour and finish concrete <br />Page 1 of 2 <br />M 1 11 N - <br />WHERE OLD FASHIONED SERVICE MEETS MODERN TECHNOLOGY! ! ! r PEI <br />.,��«,1 <br />P.O.
