0 0 Horseplay, scuffling, and other acts which tend to have an adverse influence on the safety or well being of the employees shall be prohibited. Employees shall not enter manholes,underground vaults, chambers, tanks, silos, or other similar places that receive limited ventilation unless it is determined to be safe to enter. All injuries shall be reported immediately to the foreman or superintendent so that arrangements can be made for medical or first aid treatment. Employees shall cleanse thoroughly after handling hazardous substances and follow special instructions from authorized sources. No burning, welding or other source of ignition shall be applied to any enclosed tank or vessel, even if there are some openings or until it has been determined that no possibility of explosion exists, and authority for the work is obtained from the foreman or superintendent. All tools and equipment shall be maintained in good condition. RE( Only appropriate tools shall be used for the job. Only authorized persons shall operate machinery or equipment Where appropriate, "Lock-out Tag Out"procedures shall be used. ENVIRONMENTAL HEALTH DEPARTMENT All excavations shall be visually inspected prior to backfilling to insure that it is safe to backfill. All excavations over five feet in depth shall be shored or sloped according to the State of California, CAUOSHA requirements. All excavations to which the permit applies shall be under constant competent supervision. Hard hats shall be worn in the excavation at all times. A ladder or other means of egress shall be located no more than 25 feet from all employees in the excavation. Any work performed in a manhole with any source of flammable or toxic gasses shall be done with the appropriate safety mask and rescue equipment worn at all times.
