+ e <br /> The following is an itemized list of hazardous waste violations that have not been addressed <br /> for Quick Stop Mart as of June 09, 2015. <br /> Open violations from April 28, 2015 inspection <br /> Violation#325 -Spill container failed to contain a minimum capacity of five gallons. <br /> The 87 direct-bury spill container failed when tested. All spill containers shall have a minimum capacity of five <br /> gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately discontinue <br /> deposition of petroleum into this tank until the component is repaired or replaced by a properly licensed, trained, <br /> and certified contractor under permit and inspection of the END. If the spill container can't be replaced <br /> immediately, there is a possibility that the 87 UST system may be red tagged to prevent fuel inputs. <br /> Open violations from April 21, 2015 inspection <br /> Violation#101 -No permit to operate. <br /> A permit to operate the UST system has not been issued. No person may own or operate an UST unless a <br /> permit for its operation has been issued by the local agency to the owner or operator of the UST system. <br /> Immediately obtain a permit to operate a UST system from the EHD. An "Operating without a Permit" penalty <br /> applies. <br /> Fees for the annual permit to operate the UST system have not been paid for 2015 or 2014. No person may own <br /> or operate an UST unless a permit for its operation has been issued by the local agency to the owner or operator <br /> of the UST system. Immediately contact the EHD accounting office to make arrangements for payment to obtain <br /> a permit to operate a UST system. <br /> Violation #103 -Current financial responsibility documents not submitted. <br /> Financial responsibility documents have not been submitted to the EHD. Current financial responsibility <br /> documents are required to be submitted annually. Immediately log into the California Environmental Reporting <br /> System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. <br /> Violation #107 -Plot plan/site map not submitted or failed to completely show where monitoring is <br /> performed. <br /> An accurate UST Monitoring Site Plan was not submitted. The Monitoring Site Plan submitted on CERS does not <br /> show the locations of the annular space sensors. A site plan must be submitted identifying the locations where <br /> monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> Violation #113 -Failed to submit monitoring system certification test report within 30 days. <br /> Annual monitoring system certification, leak detector testing, and spill container testing was performed on April 7, <br /> 2014 and a test report was submitted on June 16, 2014, one month late. A copy of the test report must be <br /> submitted within 30 days of the tests.
