This information was required to be submitted by May 7, 2014. This <br />information has not been received resulting in a non-compliant status for this facility. Return to Compliance paperwork <br />for the April 1, 2013 inspection was also not submitted. The outstanding violations from 2013 include: <br />- CCR 2638 Monitoring equipment certification: (a) not performed annually; (d) test report not submitted within 30 <br />days; (e) test notification not provided to the EHD at least 48 hours prior to conducting an installation, repair, <br />replacement, calibration, or certification <br />-CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months <br />The outstanding violations from 2014 include: <br />-HSC 25284 No permit to operate <br />-HSC 25404.5 Annular operating fees not paid <br />-HSC25292.2 Current financial responsibility documents not on file with the EHD <br />-CCR 2711 Current Unified Porgram Consolidated Forms not on file with the EHD (facility and tank forms) <br />-CCR 2632(d)(1)(c) Plot plan/site map not submitted or fails to completely show where monitoring is performed <br />-CCR 2712(b) Maintenance and monitoring records not maintained 3 years. <br />-CCR 2712(i) Monitoring plan, response plan, and/or operating permit and all conditions not onsite <br />-CCR 2715(c) Designated operator monthly inspection: (1) alarms not documented; (2-4) spill containers, UDCs, and <br />sumps (if necessary) not inspected; (5) required test dates not checked or noted; (6) employee training not verified <br />-CCR 2630(d) UST monitoring equipment not installed to detect a leak at the earliest opportunity <br />-CCR 2636(f)(1) Failed to maintain a continuous monitoring system for underground piping & UDCs that activates an <br />audible and visual alarm or stops the flow of product at the dispenser when it detects a leak <br />An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br />statement documenting corrective actions taken or proposing corrective actions which will be taken. Beginning January 1, 2014, ail businesses are required to submit all new (or any changes <br />to existing) UST information, including: UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST <br />Response Plan, UST Owner/Operator Written Agreement (if applicable), UST Letter from Chief Financial Officer, and <br />Owner Statement of Designated UST Operator Compliance online to the CERS at http://cers.calepa.ca.gov.
