Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility by immediately. Repeat Violation. <br /> 11. The 87 and 91 stp sump sensors were raised approximately 5 inches off the lowest point of the sump <br /> part way up the saddle and not located to detect a leak at the earliest opportunity. Monitoring equipment <br /> shall be maintained to be able to detect a leak at the earliest possible opportunity. Technician corrected <br /> this during the inspection by placing the sensor at the bottom of the sump. Continue to ensure that these <br /> sensors are at the lowest point of the 87 and 91 stp sumps. Repeat Violation. <br /> 17. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br /> of detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and <br /> certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and obtain a <br /> permit within one business day from the EHD. If the failed leak detector can't be replaced immediately, <br /> there is a possibility that the UST system may be red tagged to prevent fuel inputs. <br /> 21. Liquid was observed in the 87 and 91 stp sumps. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Technician <br /> corrected this during the inspection by removing the liquid and placing it into the HW drum on site. <br /> Continue to ensure that upon discovery, remove this liquid, make a hazardous waste determination per <br /> Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the stp sumps are <br /> maintained free of liquid.
