Monitoring system certification <br />conducted today, December 21, 2011. This inspection report supercedes the previous report. <br />Class II violations- (104, 109, 110, 111, 206, 305) <br />104. Financial responsibility documents have not been submitted to the EHD. The financial <br />responsibility documents last submitted to the EHD are dated November 13, 2009. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility by January 16, 2012. REPEAT VIOLATION <br />109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing <br />were last performed on November 22, 2010 and was due by November 2011. Testing has not been <br />completed and is 1 month past due. These tests are required once every 12 months. Immediately <br />schedule these tests and provide 48 hours notification to the EHD. Please be aware that a follow up <br />inspection will be charged at our current hourly rate. REPEAT VIOLATION <br />206. The designated operator employee training for Charan Dhillon was not current-, his training was last <br />conducted on November 23, 2007. Mr. Dhillon was observed alone on site at the time of inspection. <br />The designated operator shall train facility employees for which he or she is responsible in the proper <br />operation and maintenance of the UST system once every 12 months. An employee with current <br />training must be on site at all times. The training shall include, but is not limited to, <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by January 16, 2012. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />In Received Title: <br />EHD 23k2 R 09/01/11 Page 3 UST INSPECTION REPORT <br />
