Fax and Mail <br />Subject: Shell Food Mart, 2320 El Dorado St, Stockton, CA (SR0055104) <br />The San Joaquin County Environmental Health Department (END) has completed the review of <br />the application for permit to change the Veeder Root monitoring panel 'ILS 350 ECPU board. <br />The monitoring panel will be upgraded with an ECPU2 board with NMEM and V28 software. <br />This repair will require a "system cold start" according to the service manual. The permit has <br />beers. approvedwith the following conditions: <br />Conditions of approval: <br />1 _ Contact the local fire department, local planning or building department and the San <br />Joaquin Valley Air Pollution Control District for any applicable permits. <br />2. Any changes shall be submitted to the San Joaquin. County Environmental Health <br />Department (EHD) for approval prior to commencing work. All new addenda supersede <br />the previous ones. <br />3. Before the alarm history is cleared by the "cold start", a current printout must be <br />created and available for EHD inspection. <br />4. Contact Garrett Backus (209-468-2986) to arrange the following inspection(s) (a 48-hour <br />notice is required): <br />a. The 1311.0must witness the testing of the fimctionality of the monitoring system <br />to ensure that monitoring has been restored to meet the requirements of <br />Cali formi.a Code of Regulations Title 23 and the Health & Safety Code. All <br />monitoring system sensors must be tested. Electronic line leak detectors must be <br />tested, if installed at this site. <br />5. A "Certification of Installation / Modification" form shall be submitted by the owner <br />or the owners' agent [pursuant to (CCR) 2635(4)] before this upgrade will be considered <br />complete by EHD. <br />6. All components must be certified by an independent testing organization and labeled as <br />required by the organization. (UL, ULC, ANSI, A111, ASTM, etc. See California Code of <br />Regulations (CCR), Title 23 Chapter 16 Table B for complete list).
