2. Underground Storage Tanks — Facility Form — Provide all current information. Log into CERS, update the required information, and submit for review by the San Joaquin County Environmental Health Department (EHD). (Note: Any change of information must be updated in CERS within the days of the changes.) (The form is appended.) 3. Underground Storage Tanks — Tank Forms — Provide all current information in CERS. Complete a separate tank form for each tank. (The form is appended.) 4. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete the appended forms, and upload in CERS. This document is required to be submitted annually. 5. Underground Storage Tank Monitoring and Response Plans: Upload in CERS. Retain an approved copy of these plans on site for future reference. (The forms are appended.) 6. If you are the owner, you must submit a notification form identifying the "Designated UST Operator" for each facility owned. (A notification form is appended.) 7. Report #5021: Update all information for all programs at this site including the UST and Hazardous Waste programs, sign and date. Bring with all other forms to our accounting office to pay for transfer fees. (A form is appended.) 8. Appended for your information is a brochure of Best Management Practices (BMPs) for Retail Gasoline Outlets that you can use as part of your BMPs for your facility. 10. Hazardous Materials Management Plan: Contact EHD Hazmat Staff at (209) 468-3420 for information on updating this plan on CERS. 11. Information and instructions for obtaining a permanent California EPA ID number (The form is appended.): Complete the application for a California Hazardous Waste Permanent ID Number and submit it to the Department of Toxic Substances Control (DTSC) by fax, mail, or email. 12. Board of Equalization UST Storage Fee Account Number — Call (916) 322-9669 to obtain a current number for your facility. 13. San Joaquin Valley Air Pollution Control District — Call (209) 557-6400 to obtain an operating permit for the UST vapor recovery system. 14. Food facility operations — Call Vidal Pedraza at (209) 468-0334 for operating permit information for opening the retail food facility. A consultation appointment may be needed. 15.
