2. Underground Storage Tanks — Facility Form — Provide all current information. Log into CERS, update <br />the required information, and submit for review by the San Joaquin County Environmental Health <br />Department (EHD). (Note: Any change of information must be updated in CERS within the days of <br />the changes.) (The form is appended.) <br />3. Underground Storage Tanks — Tank Forms — Provide all current information in CERS. Complete a <br />separate tank form for each tank. (The form is appended.) <br />4. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete the appended <br />forms, and upload in CERS. This document is required to be submitted annually. <br />5. Underground Storage Tank Monitoring and Response Plans: Upload in CERS. Retain an approved <br />copy of these plans on site for future reference. (The forms are appended.) <br />6. If you are the owner, you must submit a notification form identifying the "Designated UST Operator" for <br />each facility owned. (A notification form is appended.) <br />7. Report #5021: Update all information for all programs at this site including the UST and Hazardous <br />Waste programs, sign and date. Bring with all other forms to our accounting office to pay for transfer <br />fees. (A form is appended.) <br />8. Go to: http://www.waterboards.ca.gov/cwphomelustlregulatorylindex.html rgs to obtain the current <br />statutes (Underground Storage of Hazardous Substances) and UST Regulations that you are required <br />to have for your monitoring and response plan. <br />9. Appended for your information is a brochure of Best Management Practices (BMPs) for Retail Gasoline <br />Outlets that you can use as part of your BMPs for your facility. <br />10. Hazardous Materials Management Plan: Contact EHD Hazmat Staff at (209) 468-3420 for information <br />on updating this plan on CERS. <br />11. Information and instructions for obtaining a permanent California EPA ID number (The form is <br />appended.): Complete the application for a California Hazardous Waste Permanent ID Number and <br />submit it to the Department of Toxic Substances Control (DTSC) by fax, mail, or email. <br />12. Board of Equalization UST Storage Fee Account Number — Call (916) 322-9669 to obtain a current <br />number for your facility. <br />13. San Joaquin Valley Air Pollution Control District — Call (209) 557-6400 to obtain an operating permit for <br />the UST vapor recovery system. <br />14. Food facility operations — Call Vidal Pedraza at (209) 468-0334 for operating permit information for <br />opening the retail food facility. A consultation appointment may be needed. <br />15. The most current versions of all forms must be submitted. <br />Also a list of our upcoming training classes offered to San Joaquin County businesses for FREE is <br />appended.
